How to Fill in an Application

You should be looking at the application form.  These steps are examples of what you might fill in.  Since each program has a different application, you may not have these same fields on the application you are going to use. 

 All required fields on the application are indicated by an asterisk (*).

1.   Enter [your first name] in the First Name field.

2.   Enter [your last name] in the Last Name field.

3.   Enter [your street address] in the  Address Line 1 field.

4.   Enter [your city] in the City field.

5.   Enter [your country] in the Country field.

6.   Enter [your state] in the State field.

7.   Enter [your zip code] in the Zip Code field.

If your application requires a social security number, when you enter a valid number it will also be reflected in your account information.

 

If your registration requires payment and you want to pay by online check (Bank Account Funds Transfer), you must list a Primary Phone Number with your application, in addition to an address.

8.   Enter [your phone number type] in the Phone Number Type field.

9.   Enter [your phone number] in the Phone Number field.

If you have to quit before you are finished, click Save Application.  You can come back another time and finish it by selecting Candidate Summary in the left menu bar of any screen after login.  Then you would click Resume Registration at the bottom of that screen to continue the process.

10.  When you are finished filling in the application, look below the application at the Documents Required section to see whether you need to send additional documents.

11.  Do EITHER a or b:

a.   Attach documents if you wish.  See How to Attach Documents to Your Application.

b.   Click Submit Application if no documents are required or if documents are required and you are going to mail them (postally) to CPS.  You will then indicate your location preference for the exam if one is required.  See How to Indicate Exam Location Preference for steps.
