1. Click Place a Request for Materials on the left menu bar. The Add Request for Materials screen opens and displays a list of all CPS materials available online.
2. Select [the program for which you want to request associated materials] from the Program options list.
You may
also select the All option if the materials you want are not associated
to a program.
3. Click Filter. A filtered list of materials appears.
4. Note the items and any fees or fee conditions.
5. Click [the title of] an Item Name you would like to request. The Request Details screen appears.

If a payment is not required, you will see your Request Details;
click Confirm to see a confirmation, which you may print. Your
request will be processed.
If a payment is required, do EITHER of the
following:
• Click Pay Now. The Payment screen appears. Select Credit Card or Bank Account Funds Transfer in the Payment Type field, and fill out the request information in the screen below. (Note that if you select Bank Account Funds Transfer, you may see your phone number grayed out—that’s fine, you don’t have to do anything with it. If the phone number field is blank, you must enter your phone number.) Click Submit. Your payment is processed and you are informed that it was successful. Click Continue. The confirmation of your request appears; you may print the request, if you wish, by clicking the printer-friendly page hyperlink. Your request will be processed.

• Click Pay by Check. The Material Request Confirmation screen appears telling you that your request has been submitted. A message tells you that your request will not be processed until payment is received by CPS. You can print this page by clicking the hyperlink for a printer-friendly page.
